Privacy Policy
How Preferred Residential Group Home collects, uses, and protects your information.
Preferred Residential Group Home, LLC (“Preferred Residential,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains how we collect, use, disclose, and safeguard information when you visit preferredrh.com (the “Site”) or contact us, including when you submit a referral or intake request.
1. Information We Collect
Information you provide
When you use our contact or intake forms, request a tour, or otherwise communicate with us, you may provide information such as your name, email address, phone number, organization, your relationship to a youth being referred, and details about a prospective resident. Because we provide behavioral residential care for youth, the information you share may include sensitive details about a minor. Please share only what is necessary to begin a conversation, and avoid submitting full medical or clinical records through the Site.
Information collected automatically
Like most websites, we and our service providers may automatically collect limited technical information — such as your IP address, browser type, device information, pages viewed, and the dates and times of your visits — using cookies and similar technologies.
2. How We Use Your Information
- To respond to your inquiries, schedule tours or consultations, and process intake and referral requests;
- To provide, maintain, and improve our services and the Site;
- To communicate with you about our programs, availability, and next steps;
- To comply with our legal, regulatory, and licensing obligations; and
- To protect the safety, rights, and security of our residents, staff, and organization.
3. Protected Health Information & HIPAA
Certain health information we maintain about residents may be “protected health information” governed by the Health Insurance Portability and Accountability Act (HIPAA) and applicable Arizona law. This Privacy Policy governs the Site only and is not our HIPAA Notice of Privacy Practices. Where HIPAA applies, our separate Notice of Privacy Practices controls how we use and disclose protected health information. Please do not submit detailed medical, clinical, or treatment records through this Site; we will share the appropriate secure method with you during intake.
4. Children’s Privacy
This Site is intended for use by adults — such as parents, guardians, caseworkers, and referring agencies — and is not directed to children under 13. We do not knowingly collect personal information directly from children through the Site. Information about a youth is expected to be submitted by a responsible adult. If you believe a child has provided us information through the Site, please contact us so we can take appropriate action.
5. How We Share Information
We do not sell your personal information. We may share information:
- With service providers who help us operate the Site and our communications (for example, hosting, form processing, and email providers), under appropriate confidentiality obligations;
- With clinical providers, agencies, and partners involved in a referral or placement, as needed and as permitted by law;
- When required by law, regulation, legal process, or a licensing authority, or to respond to a lawful request; and
- To protect the safety, rights, or property of our residents, staff, the public, or our organization.
6. Cookies & Analytics
We may use cookies and similar technologies to operate the Site, remember preferences, and understand how the Site is used. You can usually adjust your browser settings to refuse cookies; however, some features of the Site may not function properly without them.
7. Data Security
We use reasonable administrative, technical, and physical safeguards designed to protect the information we collect. However, no method of transmission over the internet or electronic storage is completely secure, and we cannot guarantee absolute security.
8. Data Retention
We retain information for as long as necessary to fulfill the purposes described in this Policy, comply with our legal, regulatory, and licensing obligations, resolve disputes, and enforce our agreements.
9. Your Choices
You may contact us at any time to ask about the information we hold, request corrections, or ask that we stop sending you non-essential communications. To make a request, email info@preferredrh.com or call (623) 385-2064.
10. Third-Party Links
The Site may link to third-party websites or resources that we do not control. We are not responsible for the privacy practices or content of those third parties, and we encourage you to review their privacy policies.
11. Changes to This Policy
We may update this Privacy Policy from time to time. When we do, we will revise the “Effective Date” above. Your continued use of the Site after changes are posted constitutes your acceptance of the updated Policy.
